Social Prescribing – Building a Barrier-Free Ecosystem for Active Ageing
Pioneering a healthcare boundary revolution, Taipei City Hospital has developed Taiwan’s first healthcare-led social prescribing operating system for active ageing. By integrating social prescriptions into the Hospital Information System, it transforms social participation from a welfare activity into a clinically recognized, digitally traceable health intervention. To address the challenges of a super-aged society, Taipei City Hospital introduce three core age-friendly solutions: 1.Barrier-Free Processes: A standardized medical order module enables physicians to prescribe social interventions, reducing barriers to social participation. 2.Inclusive Learning Environments: A 22-partner network transforms cultural institutions into health promotion settings, integrating IoT wearables into activities and co-developing manuals that won Gold at the U.S. TITAN Health Awards. 3.Supported Daily Living: Friendly training and care connectors support families affected by dementia, reducing isolation. The project’s greatest value lies not only in delivering social prescribing activities, but in building a healthcare-grade, culture-powered, and community- scalable infrastructure for active ageing
