Smart Integrated Care for Osteoporosis and Sarcopenia
Chiayi County, Taiwan’s most aged county, faces growing risks of osteoporosis and sarcopenia among older adults. To address these challenges, the county launched the Smart Integrated Care for Osteoporosis and Sarcopenia Program, an innovative model integrating screening, medical care, and community intervention. Through mobile healthcare services, AI-assisted osteoporosis screening, and DXA diagnosis, the program improves healthcare accessibility and screening efficiency in rural areas. An integrated care platform connects hospitals, public health centers, NGOs, and community resources to provide referral, follow-up, and continuous care. High-risk individuals receive exercise and nutrition interventions to promote healthy aging and prevent disability. More than 41,000 residents have been screened, enabling early identification and risk- stratified care. The program has reduced disability risks, healthcare costs, and caregiving burdens while generating significant social and economic benefits. Combining digital innovation, integrated care, and community empowerment, the model is scalable and replicable, contributing to SDGs 3, 4, 7, 8, 10, 11, and 17
