TIGER-Driven Healthy Aging: A Multidisciplinary SDGs Community Care Action
Taipei Municipal Gan-Dau Hospital developed the "Building a Vibrant, Long-Lived, Smart, and Livable New Community" initiative to address the challenges of an ultra-aged society. Guided by the TIGER model, the program integrates physical activity, cognitive stimulation, nutrition, chronic disease management, and technology to deliver personalized health promotion. The strategy emphasizes integration, prevention, person-centered care, and sustainability, transforming elderly care from passive service delivery to active participation. In collaboration with local agencies, academic institutions, and industry partners, the hospital has established four age-friendly community hubs supported by interdisciplinary teams and digital engagement tools. Aligned with the WHO Age-Friendly Cities and ICOPE frameworks, the program has achieved more than 42,000 visits, an 85% participation rate, and a 92% satisfaction rate, demonstrating improvements in cognition, mobility, and mental health.
