Chi Mei Medical Center

Evidence-Based Medicine and Nudge Theory to Safeguard Blood as a Public Resource
Taiwan leads globally in blood donation, yet per-capita consumption is two to three times that of Japan and South Korea—a chronic overutilization paradox. NHI data show national Cryo claims grew 6.2% annually from 2016 to 2025. The Kaohsiung Blood Center identified Chi Mei Medical Center as the highest Cryo user among five major southern medical centers. Chi Mei intervened in 2023 and halved usage by 2025, serving as a national pioneer. Applying Nudge Theory with a data- driven strategy, this project used Power BI dashboards to display usage trends and peer benchmarks, guiding ICU physicians to self-adjust prescribing, with monthly 5-minute feedback institutionalizing improvement. Mean monthly Cryo usage fell from 2,223 to 1,151 units (51%); fixed 20-unit orders dropped from 61.9% to 31.1%, with zero adverse hemostatic events. A total of 25,718 blood units were saved—benefiting 2,571 additional patients—labor reduced by 4,191 hours, and carbon emissions cut by ~103 metric tons. The approach expanded to red blood cell management (11.87% annual reduction in 2025), and a formal PBM Task Force was established, marking the transition from project action to institutionalized governance.
