Cardiac Rehabilitation: A Multidisciplinary Effort, With the Heart As Our Starting Point
TMU-Wanfang Hospital: Smart Health & Empowering Sustainability Wanfang Hospital’s digital care system connects acute to home rehabilitation, delivering holistic patient-centered care. 1.Empowering Workers (SDG 8): Turning dependence into productivity, AMI patients completing >12 sessions saw endurance surge from 4.37 to 5.18 METs. In severe cases, left ventricular ejection fraction skyrocketed from 22% to 61%, restoring full independence. This breakthrough enables successful return-to-work, slashing family burdens and societal costs. 2.Digital Governance & Safety (SDG 9): Outpatient capacity grew steadily, reaching nearly 1,000 person-times in 2024. Under standardized digital workflows, the inpatient rehabilitation intervention rate hit 100% over three years. Backed by AI monitoring, the training adverse event rate was kept at a mere 0.27% (1/363), setting a safety benchmark. 3.Healthcare Equality (SDG 10): Through robust case management, the project eliminates geographic barriers and bridges the urban-rural care gap, securing medical- center-level digital health rights for remote and mobility-impaired patients.
