Facing Taiwan's oldest demographic, Chiayi Christian Hospital (CYCH) champions "Community Inclusion" by building a seamless network from acute care to Long-term Care 3.0. Our core mission is ensuring health equity and accessible care for all vulnerable populations. Social Inclusion: We actively bridge healthcare gaps by deploying mobile clinics to remote Alishan indigenous villages. Within our hospital, we prioritize accessibility through universal design, tactile/multilingual medicine bags, and family-centered LDR rooms, creating a barrier-free environment. Tech- Empowered Care: To support patients' transition back to the community, our "Disease Case Management Platform" and AI tools (Bao-Ming fall prevention, Pump AI) drive Holistic Case Management. This accelerates recovery, allowing patients to safely return to their communities faster. Eco-Friendly Footprint: We protect our shared environment via smart surgical climate control (saving 30-40% electricity) and digital payment systems that cut 500,000 paper receipts. Recognized by the JCT Gold Award and 9th National Industrial Innovation Award, CYCH’s scalable model demonstrates profound community-centric sustainable impact.
