Hualien Tzu Chi Hospital’s “Holistic Integrated Care Program in Xiulin Township”
Hualien Tzu Chi Hospital launched the “Holistic Integrated Care Program in Xiulin Township,” Taiwan’s largest and most remote township, home to 16,871 residents—88% Indigenous and 10% elderly living alone. Due to geographic isolation and limited resources, residents face major healthcare barriers.
Since 2022, the hospital has collaborated with local health centers to build Taiwan’s first capitation-based integrated care pilot, focusing on household health profiling, risk stratification, and proactive community outreach. Services include mobile screenings, telemedicine, AI diagnostics, and culturally sensitive health education.
Key outcomes include reduced hospitalizations (−7.6%), decreased high-risk patient costs (−46.2%), and increased local outpatient visits. The program aligns with SDGs 3, 10, and 17, and has been recognized internationally. Its scalable, people-centered model is now a national reference for rural health policy.
