Cardiovascular complications and infections are prevalent, leading to frequent hospitalizations and deaths in hemodialysis patients. To address the hemodialysis population with multiple comorbidities and high complication rates, our department promotes Population Medicine through cross-team collaboration. This approach aims to prevent complications, reduce hospitalizations, and lower mortality rates by enhancing screening and early intervention through team collaboration. Currently, we focus on cardiovascular diseases and infections to reduce mortality among dialysis patients. Results include: (1) The cardio-renal team and cross-disciplinary care models have lowered the mortality rate below the national average of Taiwan, Europe, Canada, Malaysia, and the United States. (2) Nephrology and infection team interventions, such as regular nasal screening and effective decolonization strategies, have significantly reduced Staphylococcus aureus infections, earning multiple Silver Awards in national medical quality competitions. Our hospital has notably improved dialysis patients' health and survival through these targeted interventions through multidisciplinary team collaboration.
