TMUH Heart Failure Team - Advancing Multidisciplinary Care and Quality Improvement
Heart failure is a fatal cardiovascular disease that requires long-term care and treatment. Patients often experience increased rates of readmission due to a lack of disease management or inadequate treatment, leading to significant impacts on their quality of life and life expectancy. To improve treatment outcomes for heart failure patients, the Heart Failure Team at Taipei Medical University Hospital (TMUH) initiated a quality improvement project. This project, in collaboration with the American Heart Association's (AHA) Get-With-The-Guidelines (GWTG) Asia program, aimed to enhance care and outcomes for heart failure patients. Through the introduction of quality indicators and the implementation of team collaboration, the project successfully reduced readmission rates and improved the quality of life for heart failure patients. As recognition of our efforts, the TMUH Heart Failure Team was awarded the Silver Award by the AHA GWTG Asia program. This achievement signifies the team's dedication to advancing clinical practice and achieving positive treatment outcomes for heart failure patients.
